Financial Responsibility

All dental services are charged directly to the patient and he/she is personally responsible. For your convenience, this office will prepare any insurance forms or assist in making collections from your insurance company and will reimburse or credit any collections to the patient or the patient’s account. This office cannot render services on the assumption that our charges will be paid by your insurance company.
